Partnering With You to Keep Your Child Health and in Class

Our clinic's goal is simple: catch small things early, treat them quickly, and keep your child where they belong- in the classroom. Here's how we can work together this year:

Meningitis Vaccine (11th Grade Students)

If your child is in 11th grade, their meningitis vaccine must be up to date or they could be at risk of being withdrawn. If you are unsure of your child's status, Nurse Tutt can verify it for you.

Chronic Condition Action Plans

If your child manages asthma, allergies, seizures, food allergies, or another condition, an action plan from their provider helps us respond quickly and confidently- often meaning your child can stay in school instead of heading home or to the ER. Please discuss any conditions or concerns with the Nurse. 

Hearing and Vision Screening (9th Grade Students)

All 9th Grade students will be screened at school unless you return the form to decline services. If your child is referred to a specialist afterward, please return the follow-up form to us so we can make updates, accommodations, or adjustments.

Over-the-Counter (OTC) Medications

Headaches, minor aches, allergies, upset stomach- these things happen and they shouldn't have to mean missing class or school. With a signed authorization on file, we can keep basic OTC medications (like acetaminophen, ibuprofen, antacids) on hand in the clinic to manage routine symptoms right away, so your child can bet back to learning.

J. C. Lewis Mobile Health Clinic-Monthly On Campus

The mobile clinic offers check-ups, sick visits, sports physicals, immunizations, and management of chronic conditions right at school. Get the consent forms in now, and your child can be seen when needed on clinic days without missing school.

We know these forms can feel like one more thing on a long list. But every form you return is one more way we can take care of your child quickly and keep them learning. 

 We're grateful to be in this with you.  Please contact the school and speak with Nurse Tutt for the needed forms.  We can ensure that we email or have them for you in person to pick up.
